Kuesioner ini mengenai tes produk untuk minuman dalam kemasan green tea (teh hijau). Dibuat di Google Forms yang boleh Anda copy ke GDrive Anda. Ada 2 produk yang dites, dicoba secara blind (tanpa informasi merek), Sequential Monadic (Dicoba satu persatu) dengan Rotasi (urutan produk dicoba bisa berbeda antara responden satu dengan yang lain).
