Another Employee Satisfaction Survey questionnaire using Google Forms. The bulk of questionnaire is on evaluating selected list of job items/statements relevant for the company. The evaluation is usually done by using scale question asking level of satisfaction or agreement to each of the items/statements. On this questionnaire, for each item/statement additional question is asked on its importance, allowing additional analysis on item contribution towards job satisfaction or analysis on satisfaction vs importance laid out in 2 dimensional chart.
